One Tree Health (“One Tree Health,” “we,” “us,” or “our”) respects your privacy. This Privacy Policy explains what information we collect through our website at onetreehealth.com (the “Site”) and through our “Request a Callback” / patient-assistance intake form (the “Form”), how we use and share it, and the choices and rights you have.
This Privacy Policy covers information collected through our Site and Form. Separately, protected health information (PHI) that One Tree Health collects as a health care provider in the course of treatment is governed by our Notice of Privacy Practices, as required by the Health Insurance Portability and Accountability Act (HIPAA). The intake Form is designed to collect only non-clinical contact and accident-logistics information — please do not submit diagnoses, symptoms, treatment details, insurance/member IDs, or other medical information through it.

The information we collect.
a. Information you give us through the Form
When you ask us to call you about a motor-vehicle accident, we collect: your first and last name; phone number; email address; confirmation that you are 18 or older; the U.S. state where the accident occurred; the approximate date of the accident; whether you are currently represented by an attorney; your preferred contact time (optional); and how you heard about us (optional).
b. Consent records
To document the permissions you give, we record which consent checkboxes you selected, the exact wording of the consent text shown to you, the version of that text, and the date, time, IP address, and browser/user-agent associated with your submission. We keep these records to comply with applicable law (including telephone-consumer-protection requirements) and to demonstrate the consent you provided.
c. Information collected automatically
Like most websites, the Site uses cookies and similar technologies and may collect usage data such as IP address, device and browser type, pages viewed, and referring URLs. The Site uses a cookie-consent tool; you can manage non-essential cookies through the cookie settings on the Site.
d. What we do not ask for on the Form
We do not request your Social Security number, insurance or member ID numbers, date of birth, medical records, or detailed clinical history through the Form.
How we use your information.
We use the information we collect to:
- contact you — by phone call, voicemail, and SMS text message — about your inquiry and the options available to you, only as permitted by the consent you give (see Section 4);
- if you ask us to, email you the list of personal-injury law firms we work with;
- if you separately authorize it, share your information with the firms we work with so an interested firm may contact you (see Section 5);
- operate, secure, maintain, and improve the Site and Form;
- keep records of consent and communications; and
- comply with our legal obligations and enforce our Terms of Use.
We do not use the Form information to make automated decisions that produce legal or similarly significant effects about you.
Calls and text messages (your communications consent).
If you check the box authorizing contact, you are giving prior express written consent for One Tree Health to contact you at the number and email you provide, including by automated phone calls, prerecorded messages, and SMS text messages, to discuss your situation and options. Consent is not a condition of receiving any medical care. Message and data rates may apply. Message frequency varies.
You may revoke consent at any time — reply STOP to a text message, ask the caller to stop, or email PI@onetreehealth.com. We will honor opt-out requests as required by law. Reply HELP for help.
How long we keep information.
We keep Form submissions and consent records for as long as needed to respond to your request, to operate our patient-assistance service, and to comply with legal, recordkeeping, and dispute-resolution obligations, after which we delete or de-identify them.
Security.
We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit and at rest and access controls that limit access to authorized team members. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Your privacy rights.
Depending on where you live, you may have rights under laws such as the California Consumer Privacy Act (CCPA/CPRA) and other U.S. state privacy laws, including:

We will not discriminate against you for exercising these rights. To exercise a right, email PI@onetreehealth.com with your request. We will verify your request and respond within the time required by applicable law. You may use an authorized agent where permitted.
Information that is protected health information under HIPAA is generally exempt from CCPA and most state privacy laws and is handled under our Notice of Privacy Practices instead.
Children's privacy.
The Site and Form are intended for adults 18 and older. We do not knowingly collect personal information from children. If you believe a child has provided us information, contact PI@onetreehealth.com and we will delete it.
Third-party links.
The Site may link to third-party websites (for example, law-firm websites or the Site's appointment or payment tools). We are not responsible for the privacy practices of those third parties; review their policies separately.
Changes to this Policy.
We may update this Policy from time to time. We will post the updated version with a new “Last updated” date and, where required, provide additional notice.
